New Patient

Clyde Medical Practice, physicians are currently NOT taking on any new patients. Also, our walk-in clinic is currently not running.

Appointments

To book a virtual appointment, click on online booking on the home page. This might be easier than waiting for a telephone call.

Same-Day Appointments

Clydes Medical Practice does offer pre-booked family medical appointments. If you are a current patient and have an urgent medical concern that is not an emergency, please call us to book you in on the same-day available appointments. Please call for such same-day available appointments between 9 AM and 10 AM to be triaged accordingly. Spaces are limited and on a first-come, first-served basis.

Forms

Forms for employment, insurance, government or other purposes, are booked on a first come first serve basis. We would be happy to give you a letter of support if required if the forms cannot be completed before set deadlines. Please request as soon as possible to avoid unnecessary delays.

Prescriptions

For your prescriptions, our patients can send us details of medications requested by email to: medications@clydesmedical.ca. Only include your last name, year of birth &MSI number. Also, remember to include your Family physician’s name and the pharmacy you would like this sent to.

For pharmacists requesting medications or any questions about prescriptions, please send us a fax at (902) 445-4556 or send an email to medications@clydesmedical.ca, with the recommendation about protection of patient identity taken into consideration.

We recommend giving at least two weeks to allow processing of your prescriptions. Please request at least two weeks before your repeat prescriptions are due. There may be unplanned delays if this is not adhered to. Our team will contact you if there is any further information your family physician needs.

Lab Results

We do not routinely call our patients if laboratory or diagnostic investigation results are normal. You are however welcome to request an appointment to review the results if so desired.
